She had chronic renal failure and hypertension for 4 years and she had hypertensive and hypotensive attack, and irregular followup examinations. Anterior segment findings were normal except for relative afferent pupillary defect in the left eye. Fundus examination demonstrated a combined non arteritic ischemic optic neuropathy (NAION) and cilioretinal artery occlusion (CLRAO) in the left eye. The association of combined NAION and CLRAO can occur in patients with chronic renal failure and irregular systemic blood pressure. Keywords : Cilioretinal artery, ischemic optic neuropathy
