Dr. Firstly, in 2002, introduced by Fujii et all. as 25 gauge transconjunctival vitrectomy, and later in 2005, developed by Eckardt with oblique incisions; 23-gauge transconjunctival vitrectomy surgery is increasingly becoming a routine surgical procedure at most clinics nowadays owing to its practical applicability and reliability. It has been reported in studies that severe complications such as endopthalmitis in small incision transconjunctival vitrectomy do not occur at a high rate as expected and shown that this surgical technique of 23 gauge system created by oblique incisions particularly is at least as safe as 20 gauge conventional vitrectomy surgery. However, although described as a sutureless vitrectomy, the necessity of suturation in risky cases with fluid leakage of sclerotomies has been clarified in order to avoid sclerotomy related complications.
