Case of Severe Sterile Endophthalmitis After Intravitreal Bevacizumab

Dr. In this case report a severe sterile endophthalmitis after intravitreal bevacizumab injection for exudative age-related macular degeneration is presented. The patient had anterior uveitis with hypopyon, and severe vitritis that obscured fundus. He had conjunctival hyperemia and decreased visual acuity one week following 2nd dose of intravitreal injection of bevacizumab. The patient was presumed to have sterile intraocular inflammation since the vitreal cultures were negative, and the inflammation resolved with cycloplegic and topical corticosteroid therapy. Severe sterile endophthalmitis with hypopyon that mimics infectious endophthalmitis can occur after intravitreal bevacizumab. Differentiation between sterile and infectious endophthalmitis is important as unnecessary treatment in the former case and the delay in treatment in the latter case.
