A Massive Suprachoroidal Hemorrhage at the Last Stage of the Phacoemulsification Surgery and Treatment

Dr. Suprachoroidal hemorrhage (SH) is an unpredictable complication that may happen during or after any type of intraocular surgery and it is usually related with bad prognosis. The prognosis depends on the type (expulsive or nonexpulsive), localisation and amount of the hemorrhage; the patient's age and additional diseases. The primary aim of the medical and surgical therapy is to protect the entirety of eye and potential visual acuity.In this case report we present clinical findings of a glaucomatous case with suprachoroidal hemorrhage developing at the end of phacoemulsification surgery and discuss possible therapeutic options Following an uneventful phacoemulsification and foldable intraocular lens (IOL) implantation at the stromal hydration step the patient suffered from a sudden and severe ocular pain. Anterior chamber became shallow suddenly and blood oozed from the angle. IOL in the bag luxated into the anterior chamber and one haptic prolapsed from the main port with some part of iris. In order to close the wound with sutures we had to excise the exposed iris tissue and haptic. Postoperatively we detected an extensive hemorrhagic choroidal detachment by B-mod ultrasonography. We followed the case with topical steroid and cycloplegic drops for 2 weeks. SH persisted at the end of 15 days, so we performed pars plana vitrectomy, silicone oil injection, drainage of the hemorrhage from a posterior sclerotomy and explantation of the IOL. After 48 days from this surgery, the silicone oil was emulsified and there was a shallow retinal detachment with vitreoretinal band formation in the inferior quadrant.
