Still a Problem Despite Advanced Development in Ophthalmology: Retinal Siderosis

Dr., Bülent Ecevit Üniversitesi Tıp Fakültesi, Göz Hastalıkları, Zonguldak - TÜRKİYE A case of siderosis bulbi which carries iron containing metallic foreign body inside his eyes with a 30 year history has discussed in this case report. Although it is a classical and familiar complication, our purpose to discuss this case is still encountering this complication even in young patients despite todays imaging and ocular surgery techniques are in advanced levels. 43 year-old male patient was admitted with complaints of hyperemia, pain and a foreign body that extends out from his left eye. In the history, dynamite wick came to his eye but he did not undergo any operation. In a biomicroscopic examination, a foreign body was seen extending from sclera to the outside of eye at 6 o?clock line. Conjunctival hyperemia and traumatic lens opacifi cation had been appeared. Two radio-opac area was determined in his orbital computerized tomography. A foreign body that was seen from sclera was removed under general anesthesia. Traumatic cataract extraction and anterior vitrectomy was applied. In posterior segment examination, it was determined that diffuse pigmentary retinopathy was developed. In this case report, we would like to point out the necessity of performing imaging in patients with iron, copper or the foreign body that contains unkown material and if encountered, removing out the foreign body with operation without delay. Otherwise, toxic effect may cause permanent defects in retina layers and it is impossible to bring back visual lossdespite even excellent anterior-posterior segment surgeries.
