5Uz. Dr., Özel Talya Tıp Merkezi, Göz Hastalıkları Kliniği, Antalya, Türkiye Purpose: To evaluate the factors affecting anatomical and functional success in the surgical management of the retinal detachment with giant retinal tears (GRT).
Materials and methods: In this study, medical records of 75 eyes of 75 patients who surgically treated due to GRT were reviewed retrospectively. Pars plana vitrectomy (PPV) was performed in 53 patients and PPV + scleral buckling in 22 patients.
Results: Patients were divided into pairwise groups considering the presence of preoperative PVR, trauma history, macular detachment and recurrence. Final best-corrected visual acuity (BCVA) levels and postoperative visual gain were signifi cantly lower in the groups with these factors. The type of surgical procedure has no signifi cant effect on fi nal BCVA and postoperative visual gain. There were statistically signifi cant negative correlations between the GRT size, mean duration of complaints and the levels fi nal BCVA and postoperative visual gain. Trauma history, PVR, macular detachment, greater tear size were the prognostic factors associated with poor fi nal BCVA and visual gain (three- way ANOVA). PVR, macular detachment, greater tear size were found to be risk factors for the recurrence (logistic regression analysis).
Conclusions: Mean duration of complaints, tear size, trauma history, PVR, macular involvement were considered as the prognostic factors affecting the success of surgical treatment of the retinal detachment with GRT.
