PARS PLANA VITRECTOMY IN PROLIFERATIVE DIABETIC RETINOPATHY

Between March 1994 and December 1996, Pars Plana Vitrectomy was performed in 82 eyes of 70 patients with proliferative diabetic retinopathy. The mean follow up was 19 months. The operative indications, additional operative manipulations, anatomic and visual success rates, complications and additional systemic findings of all patients were retrospectively evaluated.The most frequent indication for vitrectomy was vitreous hemorrhage. Membranectomy, intravitreal SF6, intraocular silicon oil, lensectomy were done in 70, 34,4 and 4 patients respectively. Anatomic success was achieved in 57 eyes (%69.5). Visual acuity increased in 43 eyes (%52.4). The most frequent complication was retinal tear (%21.1) peroperatively and vitreous haze (%50) postoperatively. SF6, in 12 and silicon oil in 2 eyes were used as intraocular tamponade. Increase in visual acuity was obtained in %44.1 (n:15) of the eyes with very poor preoperative visual acuity and in %60.9 (n:25) of the eyes with better preoperative vision.
Our anatomic and visual succes rates are similiar with those in the literature. Low visual acuity is a negative prognostic factor except those eyes with vitreous hemorrhage. Even anatomic succes is achieved in end stage diabetic eyes which has pale optic disc, vascular sclerosis and occlusion and irreversible macular alterations, visual acuity doesn't usually improve.
