Acute retinal necrosis (ARN) is a syndrome with severe loss of vision. In this article, we discussed clinical characteristics of a patient who was followed-up and treated in our clinic. Initial clinical signs of the disease were papillitis, retinal arteritis and diffuse retinitis in peripheral retina of his right eye and its vision was at the level of light perception. He was treated with intravenous acyclovir for one week and followed by oral acyclovir for eight weeks. Three weeks after the initiation of the disease, an approximately 2-3 disc diameter area of retinal necrosis and arteritis in peripheral retina of his left eye was observed. On the third week of the disease a confluent retinal necrosis spreading from whole of peripheral retina to macula was seen in his right eye. After 24 months, the right eye was phthisic. Visual acuity of the left eye (10/10) did not change during the 24 months of follow-up and the area of retinal necrosis did not spread.
