THE EFFECT OF THE PANRETINAL PHOTOCOAGULATION ON THE VISUAL FIELD ANALYSES OF THE DIABETIC PATIENTS

Purpose: To evaluate the changes in retinal sensitivity within central 30 degrees following panretinal photocoagulation (PRP) for proliferative diabetic retinopathy. Material and Methods: One hundred eight eyes of 91 diabetic patients were studied prospectively. Those with visual acuity of 0.4 or lower and eyes with maculopathy were not included in the study. All underwent PRP in three sittings, and Humphrey field analyzer central 30-2 threshold test, with SITA-standard strategy, was done before and, 1 and 3 months after the treatment. The mean retinal sensitivity threshold (dB) and the mean deviation (dB) of the eccentries at the 3°, 9°,15°, 21° and 27° within the central 30 degree visual field were analysed both before and after PRP.
Results: The mean age of the subjects were 59.18 years, ranged 49-74 years. The best corrected visual acuity on enrollment in the study, and 1-3 months later than PRP were 0.73 ± 0.18, 0.69 ± 0.20 and 0.71 ± 0.20, respectively (p>0.05). The mean retinal sensitivities before, and 1 and 3 months after PRP were 21.25 dB, 22.68 dB and 24.09dB, respectively (p<0,05). The mean retinal sensitivity of the visual field deteriorated in the 3° and 9° eccentries, and improved significantly in the 15°, 21 ° and 27° eccentries in the first post-PRP examination at the first month (p<0,05). However, there were significant improvements in the retinal sensitivity values in all eccentries studied in the second post PRP examination at the third month (p<0,05).
Conclusion: PRP treatment has been found to provide a progressive improvement in the visual field sensitivity during a three month period follow up. Post PRP deterioration, at the first month exam, of the visual field sensitivity at the foveal and the perifoveal region was significantly improved in the third month examination.
