A 30 year-old man with bilateral severe visual loss was diagnosed as Leber's hereditary optic neuropathy which was confirmed by the presence of a mutation at 11778 position. Magnetic resonance imaging revealed hyperintense signal changes on both sides of the optic chiasm and inferior altitudinal hemianopic defect was detected on visual field testing. To our knowledge, this is the first reported case of optic chiasmal involvement on magnetic resonance imaging in the atrophic phase of Leber's hereditary optic neuropathy.
