Heavy Silicone Oil

The improvement of vitreoretinal surgical techniques and the use of silicone oil have increased the rate of successful treatment of complicated retinal detachment, including proliferative vitreoretinopathy, giant retinal tears, and severe trauma cases. Especially for superior breaks and retinal detachments, silicone oil is effective because its specific gravity is less than that of intraocular fluid. The low gravity can results in fluid accumulation in the inferior quadrants underneath the silicone oil bubble and an increased rate of proliferation in the inferior part of the retina. Heavy silicone oils are a new mixture with high specific gravity. Anatomic success rates are different in the complicated retinal detachment used heavy silicone oils. Complications are similar to those reported with conventional silicone oil. A prospective, randomized, controlled clinical trial to compare heavy silicone oils and conventional silicone oil in cases of complicated retinal detachment with involvement of the inferior quadrants will be necessary to clarify this efficacy of the heavy silicone oils.
