A CASE WITH HYPOTONY-RELATED PAPILEDEMA-MACULOPATHY AND MANAGEMENT WITH PARS PLANA VITRECTOMY

A case of persistent hypotony and visual loss is described after a penetrating eye injury and successful lens-vitreous surgery for exploration and ciliary detachment which was suspected by ultrasound but not by ophthalmoscopy because of congenital cataract in the same eye. The patient developed an intraocular pressure ranging from 4 to 6 mm Hg 2 months after the injury. She developed hypotony-related papiledema and maculopathy, and a decrease in visual acuity. During the operation, a localised ciliary detachment was detected which was caused by a fractional vitreous band. The traction was released and 2 weeks after the surgery the intraocular pressure stabilized at 12 to 14 mm Hg, and the visual acuity improved to 0.4 in 2 months with rapid resolution of papiledema and maculopathy.
