CLINICAL FEATURES AND OUTCOME OF TREATMENT IN PATIENTS WITH RHEGMATOGENOUS RETINAL DETACHMENT

Biyoistatistik Anabilim Dalı Four hundred and twenty patients diagnosed as rhegmatogenous retinal detachment and operated on by conventional retinal detachment surgical techniques at Eye Clinic of Hacettepe University Hospitals between January 1989 and June 1996, and followed up at least 6 months were studied retrospectively regarding their clinical features on admission, treatment strategies and results. Their age distribution was between 5 and 86, with a mean of 49.51 18.06 65% of them were male and 35% were female, 39% of the patients had applied within first week of beginning of symptoms. In 20% of patients no predisposing factor was determined and 20% had peripheric retinal degeneration, 20% had myopia (>6D), 15% aphakia, 14% trauma, 11% pseudophakia. More than one retinal tear and tears involving more than one quadrant were seen more often in high myopic patients and inability to find a retinal tear was more frequent in aphakic patients. Anatomic success was achieved in 87% of patients after the first operation and an increase in vision was seen in 75% of the patients. Missed retinal tears and holes during the first examination and operation were thought to be the major reason for failure to reattach the retina. In 16% of the patients whose retinas were attached after the first operation, recurrence occurred. 67.8% of the recurrent cases had grade C2 or more PVR. Anatomic and visual results were worse in those patients who .had applied late and in whom macula had been detached. Anatomic success was more in patients in whom subretinal fluid drainage and retionopexy by cryotherapy had been done. However, postoperative proliferative vitreoretionopathy was more frequent in that group of patients showing the importance of selective and appropriate usage of these techniques.
